Vision Screening

Iowa law requires any student entering Kindergarten and 3rd Grade provide the school proof of a vision screening on the Certificate of Vision Screening form provided. For students in Kindergarten and 3rd grade, screenings must occur no earlier than 1 year before enrollment and no later than 6 months after enrollment. The screening must be completed by an eye care professional. Your provider should complete the Eye Exam Section of the form, for you to return to the school office.
