Registration forms have been compiled for new and returning students. If you have questions about which forms are required for your student, please contact the main office.

New Student Forms
Forms specific to new students (including incoming preschool and kindergarten students) have been compiled below. Completed copies can be returned to the main office. In addition to the paperwork, please be prepared to provide the following documents upon enrollment:
- Copy of State Issued Birth Certificate
- Copy of Social Security Card
- Proof of Residency (copy of lease or county tax statement showing guardian's name and address)
- Copy of Student's Physical Examination from within the last 365 days
- Immunizations Records
